As you are aware, Centra Health, Inc. has mandated the COVID vaccine for all caregivers.  The term caregiver includes faculty, staff, and students.  This communication is to serve as a follow-up to the communication posted in Moodle on August 24th.  In that communication, follow-up on COVID testing, exemption forms, and uploading of documentation into VerifiedCredentials was forthcoming.  Below is the updated information on these critical items regarding the COVID vaccination mandate.

Timeline for Vaccination and Proof of Vaccination:

We highly encourage everyone to obtain the COVID vaccine. Please move forward with receiving the vaccination, as the first dose is due by October 1, 2021, and the entire series must be completed by November 1, 2021. Please upload proof of vaccination after each dose to VerifiedCredentials.

Exemption forms are located in VerifiedCredentitals for downloading, completion, and uploading.

COVID Testing:

Those that have an approved exemption are required to complete weekly COVID testing. Testing must be completed the week before starting any rotation on a Centra campus for the duration of any rotation. The first COVID test results must be uploaded to VerifiedCredentials by October 25th at noon.  Students may call HealthWorks to make an appointment for PCR testing every week.  All exemption testing is at the expense of the student. A negative test will be required to participate in the assigned clinical rotation. The results of the test must be uploaded every week on Mondays by noon (12:00 PM) to VerifiedCredentials.

Report all positive test results to Dr. Puckett (, Ms. Susan Burnette (, and Ms. Sherrie Cheryba ( at HealthWorks.  Please refer to Ms. Burnette’s Post regarding COVID Symptoms and Communication posted on August 31st.

VerifiedCredentials Guidance:

To upload documentation of proof of vaccination, exemption form, or weekly testing, you will use the attached instruction sheet with a special code to meet COVID vaccination and testing requirements.
